Equipment Loan Form
When a Beckman community member purchases an item that has the potential to be taken off campus, the requestor or custodian must complete an Equipment Loan Form.
Please help monitor that the requestor/custodian returns a completed copy to inventory@beckman.illinois.edu for final processing. The researchers who are buying equipment should also advise the Beckman inventory group when they bring it back to campus permanently or pass it on to someone else.
